Medication Pick Up - LAST CHANCE!

Greetings Families! 

If your student has medication in the school health office, and you did not make it in to pick it up by May 28, you have one more chance!  This includes all medications your student may have like inhalers, Epi pens, seizure medications, diabetic medications, daily and as-needed medications.

LAST CHANCE: ALL student medications must be picked up by Tuesday, June 9. Please call ahead to be sure someone will be available to assist. After this date, all medications left will be properly disposed of.

Note: After this June 9 date, medications CANNOT be picked up during the summer break. Expired medications left in the school health services office at the end of the school year will be destroyed.

NEXT SCHOOL YEAR:

If your student will need medication (including over-the-counter medication) again in the fall, health services will need the following:

1. a parent or guardian signature
2. a new physician order
3. a prescription-labeled container
